What is the Friends and Family Test? The NHS wants you to have the best possible experience of care. The NHS Friends and Family Test is a way of gathering your feedback, so we can continually review our service.
It is based on one simple question – “How likely are you to recommend Overseal Surgery to friends and family if they needed similar care or treatment?”
Your feedback will help us learn more about what you think of your experience – what you like and what you think we could improve.
If you would prefer to give your comments anonymously please do so.
Questionnaires are available in our waiting room, or you can download a form here, or complete the online form here. Your completed forms can be returned to us by post, email at email@example.com or placed in the ‘Friends and Family Test’ box in our waiting room.
We would also be pleased to hear any comments or queries you may have, and again a Comments, Queries, Suggestions form is available for download from our website and in our waiting room and these forms can be returned in the same way. You do not have to complete a form to let us know any comments or queries you may have, we are pleased to accept these via email at firstname.lastname@example.org, by post or telephone. Please note that if you leave us your contact details we may contact you to discuss any issues you raise.
